Registration Particulars (Provisional) for
Mulitipurpose Health Worker(F) Training/ANM

Obtain application form by sending a demand draft for Rs.10/- drawn in favour of Registrar, Tamil Nadu Nurses and Midwives Council, Chennai-4- or by Challan from this Council

Further the candidate has to submit/send the following:-

1. Filled up Application Form..
2. Copy of S.S.L.C/ book front page or Transfer Certificate.
3. Copy of S.S.L.C./10th Mark Sheet or +2 Mark Sheet
4. Original Provisional ANM Diploma Certificate with Xerox Copy
5. Registration fee of a sum of Rs.390/- (Rupees Three Hundred and Ninety Only)by means of a Demand draft drawn in any nationalised bank in favour of Registrar, Tamil Nadu Nurses and Midwives Council, Chennai-4 or by Challan from this Council. (Demand Draft should be payable at Chennai)
6. Self addressed envelope.
