Registration Particulars of Other State candidates for
Multipurpose Health Worker(F) Training/ANM:-
Register your qualification in the concern
state where you have trained. Subsequently registering your
qualification once again in other state within India where you
would like to practice nursing/higher education.
Obtain application form by sending a demand draft for Rs.10/-
drawn in favour of Registrar, Tamil Nadu Nurses and Midwives
Council, Chennai-4- or by Challan from this Council
Further
the candidate has to submit/send the following:-
1.
One Filled Application Form..
2.
Copy of S.S.L.C/ book front page or Transfer
Certificate. (One Xerox Copy)
3.
Copy of S.S.L.C./10th Mark Sheet or +2
Mark Sheet(One Xerox Copy)
4.
Original ANM Diploma Certificate with one
Xerox Copy
5
Original ANM other state Registration Certificate
with one Xerox Copy.
6.
Registration fee of a sum
of Rs.390/- (Rupees Three Hundred and Ninety Only)by means
of a Demand draft drawn in favour of Registrar, Tamil
Nadu Nurses and Midwives Council, Chennai-4 or by Challan
from this Council. (Demand Draft should be payable at
Chennai)
7.
If there is renewal system followed candidate
should renew their Registration Certificate with concerned
state Nursing Council.
