State Trained Candidates : Diploma (GNM)
Application forms are being issued on of Rs.20/-(Rupees
Twenty Only) towards the cost of two application forms (ie:one
for Nurse and other for Midwife)
Obtain application forms by sending a demand
draft for Rs.20/- drawn in favour of Registrar, Tamil
Nadu Nurses and Midwives Council, Chennai-4- or by
Challan from this Council
After getting the application form send the
following along with the filled application form.
1.
Application form - 2 filled forms
2.
Transfer Certificate or S.S.L.C.Book first
page -(Xerox) 2 Nos.
3.
Higher Secondary Mark Sheet (Xerox) - 2
Nos.
4.
Original Diploma Certificate with Two Xerox
copies
5.
Original Other state registration Certificate
with two xerox copies
6.
Original Course Completion certificate
with two xerox copies with period of training (joining
and completion date, month and year should be mentioned)
7.
Registration fee of Rs.780/- (Rupees Seven
Hundred and Eighty Only) by means of Demand draft drawn
in favour of Registrar, Tamil Nadu Nurses and Midwives
Council, Chennai-4. (Payable at Chennai)
8.
If there is renewal system followed candidate
should renew their Registration Certificate with concerned
state Nursing Council.
