Nursing as Additional Qualification:-
Obtain application form by sending a demand
draft for Rs.10/- drawn in favour of Registrar, Tamil Nadu
Nurses and Midwives Council, Chennai-4- or by Challan from
this Council.
Further
the candidate has to submit/send the following:-
1.
Application form - 1 filled form
2.
Transfer Certificate (Xerox)
3.
Higher Secondary Mark Sheet (Xerox)
4.
Original M.Sc., Degree Certificate with
one copy Xerox
5.
Original M.Sc., Course Completion Certificate
with one copy Xerox ((Joining date, Month, Year and Completion
date,month and year should be mentioned)
6.
Xerox copy of Nurse Registration Certificate
of this Council (one No.)
7.
Xerox Copy of Midwife Registration Certificate
of this Council (one No.)
8.
Xerox copy of P.B.B.Sc., Registrtion Certificate
(One No.)
9.
Registration fee of a sum of Rs.390/ (Rupees
Three Hundred and Ninety Only)by means of Demand Draft
in favour of Registrar Tamil Nadu Nurses and Midwives
Council, Chennai-4 or by Challan from this Council. (Demand
Draft should be payable at Chennai)
