with one copy Xerox (Joining date, Month, Year and Completion
date,month and year should be mentioned)

Xerox copy of Nurse Registration Certificate(1
No)
7.
Xerox Copy of Midwife Registration Certificate
(1 No)or Xerox copy of Health Visitor Registration Certificate

Registration fee of a sum of Rs.390/-
(Rupees Three Hundred and Ninety Only)by means of a Demand
Draft drawn in favour of Registrar Tamil Nadu Nurses and
Midwives Council, Chennai-4 or by Challan from this Council.
