FOR THE L0SS OF NURSE / MIDWIFE
REGISTRATION CERTIFICATE
Obtain
the Application form by sending a demand draft of a sum of
Rs.10/- (Rupees Ten only) drawn in favour of Registrar, Tamil
Nadu Nurses and Midwives Council, Chennai-4 or by Challan
from this Council.
Further
the candidate has to submit/send the following :-
1.
Application form - 1 filled form
2.
Copy of S.S.L.C. book front page or H.S.C. T.C
3.
Copy
of S.S.L.C. Mark list or H.S.C. Mark list
4.
Copy of Registration Certificate of Nurses
/ Midwife
5.
Affidavit
in a Twenty Rupee stamped paper must be obtained from
First Class Metropolitan Magistrate mentioning under which
circumstances the registration certificate was lost and
the Registgration Number, Category and date of registration
should be mentioned in the affidavit.
6.
Registration fee of a sum of Rs.390/-(Three
Hundred and Ninety only) by means of a Demand draft drawn
in favour of Registrar, Tamil Nadu Nurses and Midwives
Council, Chennai-4 or by Challan from this Council.(Demand
Draft should be payable at Chennai)
7.
A
self-addressed envelope
All
the above mentioned documents should be forwarded through
the employer where you are employed OR from the head of the
institution in which you have trained.
