AUXILIARY NURSE MIDWIFE/ MULTIPURPOSE HEALTH WORKER
Obtain the application on payment
of a sum of Rs.10/- (Rupees Ten only) drawn in favour of Registrar,
Tamilnadu Nurses and Midwives Council, Chennai or by Challan
from this Council.
Further the Candidate has to submit/send
the following:-

Filled up application form
2.
Copy of S.S.L.C. book front page or H.S.C/
T.C (2 Nos)
3.
Copy of S.S.L.C. Mark list or H.S.C. Mark
list (2 Nos)
4.
Original AUXILIARY NURSE MIDWIFE/ MULTIPURPOSE HEALTH WORKER Registration Certificate to be surrendered
to the Council.
5.
Govt.Gazette Copy for Change of Name (1
Nos)
6.
Registration fee of a sum of Rs.390/- (Rupees
Three Hundred and Ninety only) towards Change of Name
Registration fee, to be paid by demand draft (drawn in
favour of Registrar, Tamilnadu Nurses and Midwives Council,Chennai-4
,Which is payable at Chennai) or by Challan from this
Council.
