Certificate of HEALTH VISITOR
Obtain the application on payment
of a sum of Rs.10/- (Rupees Ten only) by demand draft drawn
in favour of Registrar, Tamilnadu Nurses and Midwives Council,
Chennai. (payable at chennai) or by Challan from this Council.
Further the Candidate has to submit/send
the following:-
1.
Filled up application form
2.
Copy of S.S.L.C. book front page or H.S.C/
T.C (2 Nos)
3.
Copy of S.S.L.C. Mark list or H.S.C. Mark
list (2 Nos)
4.
Original Health Visitor Registration
Certificate to be surrendered to the Council.
5.
Govt.Gazette Copy for Change of Name (1
Nos)
6.
Registration fee of a sum of Rs.390/- (Rupees
Three Hundred and Ninety only) towards Change of Name
Registration fee, to be paid by demand draft (drawn in
favour of Registrar, Tamilnadu Nurses and Midwives Council,Chennai-4
,Which is payable at Chennai) or by Challan from this
Council.
