For Changing the Name in Registration
Certificate of Nurse and Midwife
Obtain the application on payment
of a sum of Rs.20/- (Rupees Twenty only) by demand draft drawn
in favour of Registrar, Tamilnadu Nurses and Midwives Council,
Chennai. (payable at chennai) or by Challan from this Council.
Further the Candidate has to submit/send
the following:-
1.
Filled up application forms 2 Numbers
2.
Copy of S.S.L.C. book front page or H.S.C/
T.C (2 Nos)
3.
Copy of S.S.L.C. Mark list or H.S.C. Mark
list (2 Nos)
4.
Original Nurse,Midwife Registration Certificates to
be surrendered to the Council.
5.
Govt.Gazette Copy for Change of Name (2
Nos)
6.
Registration fee of Rs..780/- (Rupees Seven
Hundred and Eighty only) towards Change of Name Registration
fee, to be paid by demand draft (drawn in favour of Registrar,
Tamilnadu Nurses and Midwives Council,Chennai-4 ,Which
is payable at Chennai) or by Challan from this Council.
